Post-Activity Receipt/Document Submission Form

This form is to be used for approved Opportunities Fund applications ONLY. If you are requesting funding, please see your advisor about submitting an Opportunities Fund application during our Fall or Spring access periods. Submit your documentation indicating that they have paid for all items and/or completed the program approved by the Opportunities Fund. A list of necessary receipts will display below when you check the pertaining categories.

Deadline: Documentation must be submitted no later than the first day of classes of the semester immediately after the activity. Students who do not provide documentation by the deadline will receive a bill and be required to return all approved funding.

• A course syllabus for each course completed abroad (if not submitted with your Opportunities Fund application).
• An unofficial CUNY home campus transcript or CUNYfirst printout with your study abroad credits highlighted.
• An official airfare receipt/itinerary that includes your name, departure/return dates and cost.
• Program and housing fee receipts (i.e. Copies of the canceled checks/money orders made out to the host College).
• Receipts for any items specifically approved for (ex: passport fees, vaccination fees, ground transportation, etc.) if applicable.
• Copy of your completed Study Abroad Evaluation Form.

• Allowed file types: pdf, docx, png, jpg, jpeg
• Each document must be upright, clear, in full context, and 5MB or less.
• Pictures of documents are not acceptable. Please upload scans.
• Bank statements should have only the relevant info highlighted. Other items should be blacked out.
• Special characters in file names will cause submission errors.

• 1 page reflection paper.
• Receipts for any items specifically approved for (ex: passport fees, vaccination fees etc.).

• Receipts for any items specifically approved for (ex: transportation, program fees, etc.)
• Proof you identified as a Macaulay Honors student (ex: name badge, conference confirmation, etc.)
